Availability/Competition for training positions

With the increasing burden of chronic renal disease in the population, it is clear that the current number of trainees will be insufficient to adequately maintain the nephrology workforce.

As a consequence of this, and the recent difficulties attracting trainees into nephrology, there is likely to be good availability of training positions in coming years.

Application process

After completing the basic physician training requirements set out by the RACP, trainees may apply for advanced training in nephrology.

Trainees are currently required to apply to individual hospitals for positions and then to the SAC in nephrology for prospective approval.

Lifestyle and Practice Options

Nephrology is a demanding specialty, with high work pressures and significant on call requirements.

Many nephrologists practice in both the private and public sector, and a large proportion work only in public hospitals in capital cities, which are equipped with more resources to handle the intensive care management of patients, especially transplant cases.

There are excellent opportunities to develop your interests in many areas, including paediatrics, transplantation, dialysis, intensive care or general medicine, and research.

Nephrologists are currently not well remunerated compared to many other physicians, with a number of reforms planned to address this issue.

Intellectual content

Nephrology is an intellectually demanding specialty requiring in-depth knowledge of fluid, electrolyte and acid-base homeostasis, general, emergency and intensive care medicine, as well as other systemic diseases that can lead to kidney failure, including diabetes, cardiovascular disease and autoimmune disorders.

Nephrologists must also be confident with screening methods available, as this medical specialty relies heavily on laboratory tests, ultrasounds and CT scanning for patient management.

The doctor-patient relationship

Nephrologists spend much of their time on direct patient care. Patients of all ages are encountered and, in many cases, require long-term management.

In the case of chronic renal failure and dialysis, nephrologists get to know these patients over many years and have an appreciation of the burden on, and commitment required of, these patients and their families.

Nephrologists must be excellent communicators, as education is an important part of patient management.

The workplace

With 1 in 3 Australians at risk of developing chronic kidney disease^, renal services are frequently in demand and nephrologists are kept busy with a variety of challenging emergency cases and complicated long-term patients.

In a days work, nephrologists may see a middle-aged man with recurrent kidney stones and haematuria, a young woman with chronic urinary tract infections, a number of patients receiving haemodialysis in the hospital renal unit, and perform a renal biopsy.

Nephrologists are also often involved in research and education.

Nephrologists receive referrals from a number of sources including general practitioners, emergency departments, intensive care units and urologists.

Flexibility of training

Training is normally full time. A minimum of five full time equivalent years of certified training in accredited laboratories must be completed.

For joint training with the RACP, one year is retrospectively accredited for physician training. Part-time training may be considered in certain instances, but must average at least 8 hours per week.

The number of years of training required in these cases is determined on a pro rata basis.
